QLD Need Car Insurance Letter Explained?

I have a letter from a car insurance that I need explained (translated). I am not sure what it is supposed to mean. But, of course, there are more details but I don't know what is relevant.

Any help anyone can give me with this is greatly appreciated because it is really starting to bother me that I am pretty sure the driver told the police one thing, then their insurer another, and the insurer sent a letter to me by somehow getting my personal details from my car rego number.

If someone can let me know if they would like a copy of the letter (with personal info deleted) posted here, I can do it when I replace my printer/scanner today.
