How should I write a subpoena to get pregnancy records from the hospital?

You firstly need to seek leave of the Court to issue the subpoena. Have you done this?

I'm not sure how the pregnancy records, of a 4 year old child, could be at all relevant to a case?

Just confirming what marty says above:

1. There needs to be an active case in court to issue a subpoena

2. You need to show that the subpoena needs to be relevant to the case.
