WA Affidavit Form to Get Access to Granddaughter?

Hi,

I'm asking on behalf of a relative who has filled in an application in a case to get access to their granddaughter. Could someone tell me which is the right affidavit form to fill in to support the application? Is it a case Information Affidavit? If not what is the correct form?

Call court registry office & ask if there is a particular form required for the application being sought...
