VIC Signature Forged to Establish SMSF - What to Do?

Someone has forged my signature on the ATO documents to establish a SMSF - what can I do here? I did not witness the documents.

Start by speaking to the ATO, so far as the tax angle is concerned.
Then go to the police, with regard to the forgery aspects.
