My son has 2 daughters. The youngest is turning 6 and has not been registered for birth. The eldest is registered under the mother's name, not his and the youngest has no record. She cannot start school without a birth certificate or do anything else.

Her mother left them when she was 4 months old and has only seen them on 4 occasions up until she was 3. He does not know her whereabouts.

What are his options? Qld birth registry says he has to have the birth mother sign, though he doesn't have forms or her whereabouts.

The information available from the QLD government website states that both parents are jointly responsible for registering a child's birth. If only one parent is available to sign the birth registration application (the link to the form is below), then he or she must attach a statement explaining why the other parent is not available to sign and or difficulties in contacting them.
