I was wondering if anyone had any information or an example of what an affidavit looks like?

Is there a website that anyone knows of? Any help is greatly appreciated.

That's very open ended, as different venues and jurisdictions have different requirements for affidavits.

Someone will correct me if I’m wrong - A general affidavit is numbered statements of fact usually in sequential order or under subheadings
