VIC Affidavit Completion Errors

Hi
If an affidavit in a court case for Divorce has been lodged, can it be rejected for the following reasons: (Police Sergeant hand wrote it and said will be ok?) (Affidavit is to confirm a person has been separated for over two years)
Date at top written as month 02 and over it 05, but has clearly 05 as the month down bottom date on form
The words sworn/Affirmed- one has not been circled or crossed out

If you mean the affidavit was hand written not typed, the Courts sometimes will accept this if the applicant does not have a lawyer.

If there is a question regarding the date of separation that is a matter for you to either confirm or object.

If you are objecting, what is your reason?

I am referring that the Affidavit was typed except for the dates and sworn area. The date eg 01.05.2020 - Officer wrote 01.02.2020 and then changed the 02 to 05 which is clearly changed.
Does it matter whether the words- sworn/affirmed - was not clearly circled or crossed out.?
Not objecting, just hoping not rejected due to above.
Since is being done by telephone, will the despondent need to also attend at time on telephone to confirm affidavit?

The applicant, and affidavit is despondent verifying separation time frame by current partner
