Hello, I am representing myself at family court and need to file a subpoena for my ex's police records.
My question is, what do I need to include on that subpoena? More importantly, what's needed?

I assume his name, dob and address.

I have attached a screenshot of the section I am referring too

Should I include case numbers if I have them?

Looking for a specific report. I guess explaining why on here in public post is not suitable. Having said this, I am seeking a particular incident, not fishing.
