QLD Letter of administration

Hi I have to get a Letter of Administration I have advertised on the ICLRQ my question is what forms do I have to fill out ?Do I have to send the one to the public trustee with a copy of death certificate and the other forms to Brisbane Supreme Court also can they be emailed.
