QLD How to Apply for Changes to Spousal Maintenance?

I wish to lodge an application to the Family Court to have a Spousal Maintenance Consent Order from 2002 changed. What documents should I file?

I understand that I have to file an Application on a form, and an Affidavit. Should I also file a reason for the Application?? If so, where?

Is Spousal Maintenance extinguished by the remarriage of the receiving partner?
