Ok Rod thanks. Someone from the court replied in a vague manner regarding what address I should use.

How do I subpoena her employer for her home address?

Thanks in advance again.

Worth a try asking the other party first.

A subpoena is an order you request from the court. Ask them what form you need.

With respect to service costs, the court will usually have a set allowance. What it actually costs you may differ, but you can't claim more than the allowance.
