Hi,

I've finalized my affidavit for an initiating application and I have a number of attachments/Annexures as supporting documents referred to in this (which are screenshots of text messages, photographs etc).

Do I attached these and lodge these as one file with the affidavit?

Also, do all pages of the affidavit statement need to be signed/witnessed (including the annexures?) or???

Yes..

Are you filing in FCCoA? If so Rule 15.28 applies to annexures, in particular >>>
(3) An annexure must:

(a) be paginated; and

(b) bear a statement signed by the person before whom the affidavit is made identifying it as the particular annexure mentioned in the affidavit.
