Any answers you get here will be a combination of (a) know it off the top of their heard, and (b) a quick check of something to check a particular circumstance. To answer your questions will involve some considerable research. The first part, possibly not so much. But to track down each and every decision? Assuming they are reported, even just providing you with the links is going to take some time. Good luck finding someone who is willing to do that for nothing.
