I already submitted the divorce application as we were separated for more than 12 months. I wonder if the effective date for the divorce will happen exactly after 12 months of separation or if it happens after the judge makes a decision.

I would not worry too much. If it was a joint application it will be accepted. If it was a sole application then it is up to the ex to dispute it. If the ex doesn't dispute it, then it won't matter

The divorce will be finalised one month and one day after the court hears and decides the application.

Thus, for example, if you separated on 1 November 2014, applied for a divorce on 1 November 2015 and you were given a hearing date for 30 November 2015, then the divorce (if granted) will become final on 31 December 2015.
