I have found out that I have a son. However, the mother had another man's named placed on the birth certificate. I have DNA testing evidence and the other man has agreed to sign anything. What form do I lodge in order to have his name removed, mine added and my son's last name changed to mine?

I would hold off on doing that until you get court orders granting you access to your son, otherwise you might have a son in name only. So he is your kid, but mum won't let you see him and then you're also making yourself liable for child support.

I'm not sure but give births, deaths and marriages in your state a call. If mum is agreeing it might be a relatively easy thing to do and not need court involvement.

I already have access to him, and have done for about 2 years. Just never had the time or money to sort it until now
