If I post something online that is public - for example that a fictional man called John... John has serious mental health issues, even though they have been diagnosed in real life and I know this, can I still get in legal trouble given that the general public does not know this diagnosis?

Not enough information to form an opinion.

And why would you make public someone's private medical condition?

Say if you're seeking help on an Internet forum that is public or a Facebook group. For example, say there's a bipolar group and you ask "X has been diagnosed with bipolar, what can I do when they do?"

As long as you do not disclose the identity of 'X' either directly or indirectly you are OK.
