Hi there I have a soon to be 6 year old his mum has come and gone like a yo-yo over the past 4 years and she has now told me her life is to hard to have him every second weekend and has asked me to get forms as she wants to hand over her parental rights can someone help me I don’t no where to get such forms as she will also be leaving the country

There is no such form. She can't relinquish parental responsibility. It can be taken from her by a court but only IF the court deem her to be danger to the kid.

Your best bet is just to remain in contact with her in-case you need her signature. Realistically, the only thing you might need her to do is sign a passport form. Apart from that you can do everything else on your own
